Based on the most recent data for each metropolitan area, Fair Market Rents (FMR) generally rise and fall annually. As such, we expect the award amount to vary up to 10% each year based on the current FMR for our community, not the FMR for the year in which the application was submitted. The acceptance of $209,040 from Anthem Blue Cross Blue Shield Healthcare Solutions, $104,520 from Silver Summit Health Plan, and $313,560 from United Health Care / Health Plan of Nevada is applied to the contract with HELP of Southern Nevada (CBE No. 605522-19 for Healthy Living for Medically Fragile Individuals/Families Experiencing Homelessness). |
BACKGROUND:
Clark County Social Service (CCSS) requests that the Board of County Commissions accept the renewal of award from the U.S. Department of Housing and Urban Development (HUD) for Healthy Living Consolidated Project (HLCP) to provide permanent supportive housing to households experiencing homelessness that are medically fragile; authorize the continuation of one (1) grant funded position; authorize the Assistant Director of Social Service or their designee to sign the grant documents; approve the acceptance of program funds from Managed Care Organization partners; and accept any grant funds awarded.
On January 29, 2021, the U.S. Department of Housing and Urban Development (HUD) awarded $2.5 billion to support local homeless assistance programs across the nation. Due to the pandemic, HUD was given congressional approval to renew 2019 CoC projects instead of hosting a 2020 CoC competition. The renewal amounts differ slightly from the previous award due to changes in local fair market rents (FMR).
Clark County Social Service (CCSS) has previously applied for the Healthy Living Consolidated Project through the Southern Nevada Homelessness Continuum of Care (CoC) consolidated grant application process which was submitted to HUD in 2019. The renewal award reflects an increase to FMR.
HLCP provides 133 units of housing to individuals and households experiencing homelessness that are medically fragile and are referred from hospitals, managed care organizations or other providers. Stable housing placement and intensive case management is provided in coordination with match and leveraged resources for medical support and access to addiction and mental health counseling to build toward self-sufficiency and increase client success. HLCP is a public-private partnership between CCSS serving as the fiscal agent, HELP of Southern Nevada providing the client service delivery, and three (3) Managed Care Organizations (MCO) (Health Plan of Nevada, Anthem, and Silver Summit) providing medical case management and supplementing the grant funds for client service delivery and supportive services.
The MCOs completed a cost savings analysis for their members to validate and justify their need to support case management and services for clients in this program. Their contribution to the program provides case management, supportive services, and administrative support for this program.
HLCP has been renewed for $1,834,448 for twelve (12) months of services with a project term of October 1, 2021 through September 30, 2022, and the grant includes annual renewal options through the local CoC process. HLCP will receive cash match resources, up to $627,120 from the partnering MCOs based on the number of MCO clients they serve and cash match resources of $200,000 from the CCSS property tax revenue.
