BACKGROUND: 

The Clinical and Community Services Department was added to the County's Hybrid Entity through   
the Fifth Amendment to the Resolution Designating the HIPAA Hybrid Entity Status, 4-1-03. Clark County developed a Notice of Privacy Practices for Clinical and Community Services for approval.
