BACKGROUND:
Social Service modeled Project HOME after the Step-Up program model. The Step Up program is highly successful in providing transition aged youth (TAY) with the needed support to develop self-sufficiency skills and be a productive member of our community.
Over the last seven years, the number of youth (13-24) that have tested positive for HIV has remained consistently high, ranging from 74-89 persons annually, which is roughly 20% of all new HIV positive diagnoses each year. To provide the needed assistance and support to TAY with HIV, Project Home was launched in 2022 as an expansion of Step Up. Step Up and Project HOME were supervised by one person. Due to the success of Project HOME, a separate supervisor for each program is needed.
The Project HOME supervisor position is funded through the federal grant, Ending the HIV Epidemic. This position will ensure clients receive case management, peer support and navigation services, classes, option to participate in focus groups, support groups and other services targeted to each client's unique needs. This also includes a heavy emphasis on the importance of medication adherence, making and keeping medical appointments, and maintaining an undetectable viral load.
