The initial Contract amount was $3,233,600. Amendment No. 1 increased the Contract amount by $1,010,500.01, Amendment No. 2 increased the Contract amount by $808,400, Amendment No. 3 will increase the Contract amount by $721,275, for a new total Contract amount of $5,773,775.01. |
BACKGROUND:
On September 19, 2017, the Board of County Commissioners (Board) approved the award of CBE No. 604572-17 for Nursing Services to Foundation of Positively Kids, A Nonprofit Corporation (NBE) in the amount of $3,233,600 for nursing services at Child Haven Campus for Southern Nevada's severely disabled, medically fragile and/or developmentally delayed children and their families.
The following are the pertinent provisions of the Contract:
• The term of the Contract was from date of award through June 30, 2018 and included the option to renew for 3, one-year periods. There are no renewal options remaining.
• Included the County’s option to terminate for cause, convenience or default and suspend the Contract with advanced written notice.
Amendment No. 1 extended the term of the Contract through September 30, 2022.
Amendment No. 2 extended the Contract term through September 30, 2023.
Approval of Amendment No. 3 will provide for an extension of the Contract term for 9 months, on a month-to-month basis through June 30, 2024 and a rate increase for On-Site Qualified Registered Nursing Services. The attached Amendment No. 3 document details the modifications and costs involved.
Foundation for Positively Kids, A Non-Profit Corporation currently holds a Clark County Business License.
