BACKGROUND:
On September 15, 2020, the Board of County Commissioners (Board) approved the award of RFQ 605645-20 for In-Home Safety Services to Apple Grove Treatment Center LLC dba Apple Grove Foster Care Agency (WBE), Eagle Quest (NBE), Shining Star Community Services, LLC dba Shining Star Community Services and Shining Star (WBE), Southwest Integrated Care Services LLC (MBE), and Youth Advocate Programs, Inc. (NBE) in the amount not to exceed $5,000,000 for In-Home Safety Services.
The following are the pertinent provisions of the Contract:
• The term of the Contract was from date of award through June 30, 2021 and included the option to renew for 4, one-year periods and included a 4 month extension. There are no renewal option(s) remaining.
• Included the County’s option to terminate for cause, convenience or default and suspend the Contract with advanced written notice.
Approval of Amendment No. 1 will provide for two additional one-year renewal periods, update the Notices section, and update Exhibit A. The attached Amendment No. 1 document details the modifications involved.
Apple Grove Treatment Center, LLC dba Apple Grover Foster Care Agency, Shining Star Community Services, LLC dba Shining Star Community Services and Shining Star, and Youth Advocate Programs, Inc. currently hold Clark County Business Licenses.
Eagle Quest and Southwest Integrated Care Services LLC currently hold Clark County Vendor Registrations.
