Healthy Living for Medically Fragile Individuals/Families Experiencing Homelessness

Additional Comments:

The initial Contract amount was not to exceed $3,221,735. Amendment No. 1 increased the Contract by an amount not to exceed $6,216,820. Amendment No. 2 will increase the Contract by an amount not to exceed $1,363,718.42, for a new total Contract amount not to exceed $10,802,273.42 should all renewal options be exercised.

 

BACKGROUND: 

On December 3, 2019, the Board of County Commissioners (Board) approved the award of CBE No. 605522-19 to HELP of Southern Nevada (NBE) in the amount of $3,221,735 for Clark County to assist with the cost of operating intensive case management with permanent housing for medically fragile individuals/families experiencing homelessness through the Healthy Living Program.

The following are the pertinent provisions of the Contract:
                     The term of the Contract was from December 3, 2019 through September 30, 2020, and included the                      option to renew for 4, one-year periods. There is 1 renewal option remaining.
                     Included the County’s option to terminate for cause, convenience or default and suspend the Contract                      with advanced written notice.

Amendment No. 1 expanded services and increased the number of clients receiving services.

Approval of Amendment No. 2 will provide for an increase of funding as Department of Social Service received additional grant funds from HUD for the continuation of a part-time position and to sustain the increase of Fair Market Rents (FMR). The attached Amendment No. 2 document details the modifications and costs involved. 

HELP of Southern Nevada currently holds a Clark County Business License.
