BACKGROUND:
On November 5, 2013, the Board of County Commissioners (Board) approved the award of CBE No. 603154-13 to Lisa A. Durette MD PLLC dba Healthy Minds (WBE) in the amount of $8,712,758 to provide comprehensive mental health services for children served by the Department of Family Services (DFS).
The following are the pertinent provisions of the contract:
• The term of the contract was from December 13, 2013 through June 30, 2015 and included the option to renew for 9 one-month periods. There are no renewal options remaining.
• This contract included the County’s option to terminate for cause, convenience or default and suspend the contract with advanced written notice.
Amendment No. 1 provided for expanded clinical service provision, data collection and reporting requirements, and extended the Contract period through June 30, 2017.
Amendment No. 2 extended the term of the Contract through December 30, 2017, with the option to renew for 3 one-month periods.
Amendment No. 3 extended the term of the Contract for up to 6 months through September 30, 2018.
Amendment No. 4 extended the term of the Contract for 3 months through December 31, 2018 with the option to renew for one three 3-month period. The extension incorporated additional services to provide Basic Skill Training as outlined by State of Nevada Division of Health Care Financing and Policy Federally Qualified Health Centers and incorporated non-exclusivity of service.
Amendment No. 5 extended the term of the Contract for 3 months through June 30, 2019, with the option to renew for one 3-month period.
Amendment No. 6 extended the term of Contract for 3 months through December 31, 2019, with the option to renew for three 3-month periods.
Amendment No. 7 extended the term of the Contract for 3 months through December 31, 2020.
Approval of Amendment No. 8 will extend the term of the Contract through December 31, 2021. The attached Amendment No. 8 document details the costs involved.
