BACKGROUND:
Clark County Social Services (CCSS) requests that the Board of County Commissioners (BCC) ratify and authorize the submission of the Clark County Street Medicine grant application to the U.S. Department of Housing and Urban Development (HUD) in the amount of $561,338 for the period July 1, 2027, through November 30, 2028, for Healthcare in Action, Inc. to provide medical outreach and supportive services to individuals experiencing homelessness; authorize the County Manager, or his designee, to sign all related documents, accept any funds awarded, approve subgrant to Healthcare in Action, Inc. for $541,338 and authorize the establishment of one (1) part-time hourly Management Assistant, or equivalent position, contingent on grant award from HUD for the period the grant funds are available.
Clark County Street Medicine (CCTH) is a new project that was submitted in response to the 2026 Notice of Funding Availability for the Southern Nevada Homelessness Continuum of Care (CoC) that was re-released on September 18, 2026, with a due date of September 30, 2026. On September 23, 2026, the CoC Board approved the priority listing which included this project in tier 2. Due to the shortened timeframe of this solicitation, CCSS requests ratification of the submission.
Per the expectations set by the CoC, only applicants that attended the mandatory training on June 24, 2026, were eligible to apply in the local solicitation. Healthcare in Action was unable to attend and asked CCSS to be the fiscal agent as CCSS was eligible to apply. As the fiscal agent, CCSS will retain $20,000 in administrative funds and provide a $5,000 in-kind match. Healthcare in Action will receive a subgrant award of $541,338 and will provide the remaining match requirement of $135,335. The total program budget is $727,588.
Clark County Street Medicine will provide mobile medical and supportive services to individuals and families experiencing homelessness, with the goal of improving medical conditions and helping clients transition to permanent housing as part of a housing program. Connections to services are provided in coordination with wraparound case management and supportive services to increase client success and movement toward self-sufficiency.
CCSS is applying for $561,338 for an initial seventeen (17) months of services with a project term of July 1, 2027, through November 30, 2028. Renewals for the project will be on an annual basis with the first renewal period of December 1, 2028 - November 30, 2029 This item has been approved as to form by the District Attorney's office.
