The initial Contract amount was $3,289,275, Amendment No. 1 increased the Contract amount by $54,821.25, Amendment No. 2 increased the Contract amount by $620,000, a funding increase was approved in the amount of $77,694.41, Amendment No. 3 will increase the Contract amount by $759,855, for a new total Contract amount of $4,801,645.66. |
BACKGROUND:
On March 6, 2018, the Board of County Commissioners (Board) approved the award of RFP No. 604609-17 for Comprehensive Professional Mental Health Services for Juvenile Assessment Center to Eagle Quest (NBE) in the amount of $3,289,275 to provide a coordinated site which contributes to the safety for youth, families, and the community through early intervention.
The following are the pertinent provisions of the Contract:
• The term of the Contract was from date of award through December 31, 2018 and included the option to renew for 4, one-year periods and included a 6 month extension. There are no options remaining. • Included the County’s option to terminate for cause, convenience or default and suspend the Contract with advanced written notice.
Amendment No. 1 corrected the Contract title, the annual payment amount, and included a one-time set up cost of $54,821.25.
Amendment No. 2 provided 4 family resources specialists to operate Harbor Mobile and Rural Services in outlying communities and extends the Contract term through June 30, 2023.
Approval of Amendment No. 3 will add an additional one-year renewal to the contract term and include a monthly increase to cover operations of the program. The attached Amendment No. 3 document details the modifications and costs involved.
Eagle Quest currently holds a Clark County Vendor Registration.
