Additional Comments:

The Contract amount was $493,391.25 for initial term and $657,855 annually thereafter.  Amendment No. 1 will increase the Contract amount by $54,821.25 for a new total Contract amount of $3,179,632.50 should all renewal options be exercised.

 

BACKGROUND: 

On March 6, 2018, the Board of County Commissioners (Board) approved the award of RFP No. 604609-17 to Eagle Quest (NBE) in the amount of $657,855 annually to provide Comprehensive Professional Mental Health Services for the Juvenile Assessment Center. 

The following are the pertinent provisions of the Contract:
•                     The term of the Contract was from date of award through December 31, 2018 and included the option to                      renew for 4, one-year periods. There is one renewal option remaining.
•                     Included the County’s option to terminate for cause, convenience or default and suspend the Contract                      with advanced written notice.

Approval of Amendment No. 1 will correct the Contract title, the annual payment amount, and will include a one-time set up cost of $54,821.25 which was left out of the Contract in error.  The attached Amendment No. 1 document details the modifications and costs involved.

Eagle Quest currently holds a Clark County vendor registration.
