The initial Contract amount was $1,314,102 annually, Amendment No. 2 will have no fiscal impact and a total Contract amount of $ 6,570,510 should all renewal options be exercised. Contract amounts are subject to change based on the volume of participants in the program, which is controlled by the courts, and upon approved budget appropriations. |
BACKGROUND:
On September 4, 2018, the Board of County Commissioners (Board) approved the award of RFQ No. 604856-18 to Freedom House Sober Living dba Freedom Behavioral Health (NBE), Lisa A. Durette, N.D., PLLC dba Healthy Minds (WBE/VET), and Choices Group, Inc., a Division Of Pathways (NBE) in the amount of $1,314,102 for outpatient drug treatment services for participants in District Court Specialty programs.
• The term of the Contract was from date of award through June 30, 2019 and included the option to renew for 4, one-year periods. There are 2 renewal options remaining.
• Included the County’s option to terminate for cause, convenience or default and suspend the Contract with advanced written notice.
Approval of Amendment No. 1 allowed the Department of Family Services (DFS) to join the contract and pay the providers directly for Treatment Services provided to DFS clients recommended by the Eighth Judicial District Court.
Approval of Amendment No. 2 will include grant flow-downs. The attached Amendment No. 2 document details the modifications involved.
Freedom House Sober Living dba Freedom Behavioral Health, Choices Group, Inc., a Division of Pathways, and Lisa A. Durette, N.D., PLLC dba Healthy Minds currently holds a Clark County Business License.
