The initial Contract amount was $1,174,102 annually and collectively, Amendment No. 1 increased the Contract amount by $140,000 annually for a new annual Contract of $1,314,102, Amendment No. 2 had no fiscal impact, Amendment No. 3 will have no fiscal impact, for a total Contract amount of $6,570,510 should all renewal options be exercised. Contract amounts are subject to change based on the volume of participants in the program, which is controlled by the courts, and upon approved budget appropriations. |
BACKGROUND:
On September 4, 2018, the Board of County Commissioners (Board) approved the award of RFQ No. 604856-18 to Freedom House Sober Living dba Freedom Behavioral Health (NBE), Lisa A. Durette, MD, PLLC dba Healthy Minds (WBE/VET), and Choices Group, LLC (NBE), in the amount of $1,174,102 annually, to provide outpatient drug treatment services for participants in Specialty Court programs.
The Contract with Freedom House Sober Living dba Freedom Behavioral Health was Terminated for Convenience under Section IX: Suspension and Termination, paragraph B.2 on July 28, 2020 due to change in service types provided.
The following are the pertinent provisions of the Contracts:
• The term of the Contracts were from date of award through June 30, 2019 and included the option to renew for 4, one-year periods. There are 2 renewal options remaining.
• Included the County’s option to terminate for cause, convenience or default and suspend the Contract with advanced written notice.
Amendment No. 1 allowed for the inclusion of the Department of Family Services (DFS) as a user of the Contracts and for then to pay providers directly for services provided to DFS clients.
Amendment No. 2 included grant flow-downs for DFS.
Approval of Amendment No. 3 will provide for additional service types to the Eighth Judicial District Court. The attached Amendment No. 3 document details the modifications and costs involved.
Lisa A. Durette, MD, PLLC dba Healthy Minds and Choice Group, LLC currently hold a Clark County Business License.
