Additional Comments:

The total annual cost for the positions shall start at $138,372.00 including salary and benefits.  The position will be supported through existing grant funding from Ending the HIV Epidemic (EHE).

 

BACKGROUND: 

The Step-Up program model is highly successful in providing transition aged youth (TAY) with the needed support to develop self-sufficiency skills and be a productive member of our community. The program includes case management, updates on goals, education, employment, parenting and developing/maintaining healthy personal habits.  One of the most important aspects of the program is youth knowing they have a trusted adult to share concerns, hopes, plans and missteps. 
 
Over the last five years, the number of youth (13-24) that have tested positive for HIV has remained consistently high, ranging from 74-89 persons annually, which is roughly 20% of all new HIV positive diagnoses each year. To provide the needed assistance and support to TAY with HIV, Project HHHOME will be an expansion of Step Up specifically to serve youth ages 16-24 who are living with HIV/AIDS.  Project HHHOME will take the successful Step Up model and provide the same developmentally appropriate, targeted services to TAY (ages 16-24) with HIV/AIDS.

The services provided shall include case management, peer support and navigation services, classes, focus groups, support groups and other services targeted to each client’s unique needs. This will also include a heavy emphasis on the importance of medication adherence, making and keeping medical appointments, and maintaining an undetectable viral load.  Project HHHOME will be supervised by the current Step Up Social Work Supervisor.
