The following are the pertinent provisions of the Contract:
The term of the Contract was from July 1, 2019 through June 30, 2022 and included the option to renew                      for 2, one-year periods. There are 2 renewal options remaining.

Amendment No. 1 provided for the addition of 4.2 Certified Nursing Assistant positions, 1 Mental Health Professional position and 2 Registered Nurse positions.  As part of the negotiation of the amendment, Wellpath, LLC agreed to share the cost of the Mental Health Professional and Registered Nurse positions and paid for these positions on their own from July 1, 2020 through December 31, 2020.  In addition, Wellpath, LLC agreed to defer 1.5% of their 3% allowable price increase for the current contract year to the next contract year beginning July 1, 2021.  The attached Amendment No. 1 document details the modifications and costs involved.

Approval of Amendment No. 2 will adjust Provider Responsibilities, add a Pharmacy program utilizing 340B pricing, and replace in its entirety page 27 of Attachment 1 and page 30 of Attachment 3 in Exhibit A. The attached Amendment No. 2 document details the modifications and costs involved.
