Description:

Healthy Living Rapid Rehousing

Additional Comments:

Based on the most recent data for each metropolitan area, Fair Market Rents (FMR) have risen approximately 20%. As such, we have updated the award amount to reflect the change from the FY2022 FMR used in the application to the FY2023 FMR HUD will use to award project funds. As FMR changes we expect the award amount to vary each year based on the current FMR for our community, not the FMR for the year in which the application was submitted.

 

BACKGROUND: 

Clark County Social Service (CCSS) requests that the Board of County Commissioners ratify the submission of Healthy Living Rapid Rehousing (HLRRH) Project, a 2022 renewal  application for the Southern Nevada Homelessness Continuum of Care (SNH CoC) due on September 30, 2022 to the U.S. Department of Housing and Urban Development (HUD); authorize the Administrator of Human Services to sign all grant documents; accept any grant funds awarded; and authorize the continuation of one (1) Part Time Hourly Management Assistant or equivalent position, contingent on award from HUD.

HLRRH is a rapid rehousing (RRH) project written in response to the 2022 Notice of Funding Opportunity for the CoC, and approved by the SNH CoC Board on September 15, 2022, for inclusion in the 2022 SNH CoC consolidated application to HUD. HLRRH ranked 3 out of 27 in the local scoring and ranking process with $735,595 in Tier 1 of the consolidated application priority listing.

HLRRH provides 58 units of housing to individuals and households experiencing homelessness that are medically fragile and are referred from hospitals, managed care organizations or other providers. Stable housing placement and intensive case management is provided in coordination with match and leveraged resources for medical support and access to addiction and mental health counseling to build toward self-sufficiency and increase client success. HLRRH is a public-private partnership between CCSS serving as the fiscal agent, HELP of Southern Nevada providing the client service delivery, and three (3) Managed Care Organizations (MCO) (United Health Care/Health Plan of Nevada, Anthem Blue Cross and Blue Shield Healthcare Solutions, and Centene/SilverSummit Healthplan,) providing medical case management and supplementing the grant funds for client service delivery and supportive services.

CCSS is applying for $735,595 for twelve (12) months of services with a project term of April 1, 2024 through March 31, 2025, and the grant is eligible to apply as a renewal project annually through the local CoC process. The $735,595 amount reflects the current totals based on the 2022 FMR used in the application. The final grant award amount will be calculated based on increased 2023 FMR and is anticipated to total $885,391.  HLRRH will partner with HELP of Southern Nevada through a contract developed specific to this project. HLRRH will receive cash match resources of $601,808 from the partnering MCOs. Grant administration funds will be used to support one Part Time Hourly Management Assistant to conduct monthly quality assurance reviews and support the work of this grant.
