The initial Contract amount was $866,400. Amendment No. 1 decreased the Contract amount by $10,000. Amendment No. 2 increased the Contract amount by $855,600. Amendment No. 3 increased the Contract amount by $800. Amendment No. 4 will increase the Contract amount by $854,400 for a new total Contract amount of $2,567,200. |
BACKGROUND:
July 19, 2023, in accordance with the NRS 332 Resolution approved by the Board of County Commissioners (Board), the Department of Finance, Purchasing and Contracts Division approved the award of CBE No. 606695-23 to New Hope Placement, LLC dba Well Care Living (MBE) in the amount of $866,400 for a housing and crisis stabilization navigation center to serve high acuity homeless adults without children to stabilize their lives and move towards independence.
The following are the pertinent provisions of the Contract:
• The term of the Contract was from date of award through December 31, 2023.
• Included the County’s option to terminate for cause, convenience or default and suspend the Contract with advanced written notice.
Amendment No. 1 removed travel from Exhibit A Scope of Work and reduced the Contract amount by $10,000.
Amendment No. 2 added a 6-month extension for the continuation of services while an RFQ is being developed.
Amendment No. 3 increased the Contract amount by $800 .
Approval of Amendment No. 4 will extend the Contract for an additional 6 months, on a month-to-month basis. The attached Amendment No. 4 document details the modifications and costs involved.
New Hope Placement, LLC dba Well Care Living currently holds a Clark County Business License.
